It is the policy of The Center for Wholeness and Wellness Health Services Clinic to maintain the medical files of full-time enrolled students for seven years either after graduation or last patient contact. After that time all medical records are shredded, in accordance with HIPAA regulations.
To request your medical records, please download our Authorization to Release Medical Records form. You will need to carefully complete the form, making sure to sign the Patient Authorization and either mail, fax, or email to:
